7 simple steps to your first appointment

As our practice has become all-digital, we are happy to provide you with a streamlined, online way to complete all the usual paperwork. Please read the instructions carefully.

Step One

Fill out the Appointment Request Form (Unfortunately, we do not schedule appointments over the phone)

Step Two

Our insurance department verifies your benefits and what your patient responsibility will likely be. At the same time, our clinical staff reviews your request to confirm that our practice is the right fit for you.

Step Three

Our Intake Coordinator will reach out to you for any clarifications, and then will assign you to the appropriate therapist.

Step Four

You will receive a call from your therapist to schedule an initial appointment, after which you will receive an email confirmation of the appointment.


Instructions for completing the rest of your paperwork

(These are the final steps that need to be completed prior to your first appointment.)

Step Five

Follow the link in the confirmation email to log into your Patient Portal. Here are the detailed instructions and a video tutorial.

Step Six

Once inside the portal, please complete the Consent to Treatment and Payment Authorization forms.

Step Seven

Once your forms are done, your therapist will contact you with details about your meeting.

Once your treatment has begun, we will submit your sessions to insurance, if applicable, and send you an invoice each month with your financial responsibility.
